For patients: nothing. Your therapist enrolls you and insurance covers the cost through RTM (Remote Therapeutic Monitoring) billing codes. For therapists: the platform generates more revenue than it costs — RTM billing adds ~$90+/month per patient. During beta, it's free for both therapists and patients.
Yes. We are fully HIPAA-compliant with end-to-end encryption, BAAs with all infrastructure providers, and a therapist-governed data model. Your data is never sold, never mined for marketing, and never shared with anyone except your therapist. This is the opposite of what BetterHelp and Talkspace have done.
ExtendedSessions.ai is not a crisis intervention tool and does not provide emergency mental health services. However, it has built-in safety protocols: if the AI detects signs of distress, it immediately surfaces 988 Suicide & Crisis Lifeline information, alerts your therapist, and guides you toward appropriate emergency resources. It never attempts to handle a crisis on its own.

AI Disclosure: All patients are clearly informed that they are interacting with an artificial intelligence system, not a human, before they can access any AI features. This disclosure is reinforced through a mandatory comprehension quiz during onboarding and repeated at regular intervals during use.
Crisis Detection & Response: The platform uses a two-layer detection system combining keyword pattern matching and AI-based semantic analysis to identify signs of suicidal ideation, self-harm, or acute distress. When a potential crisis is detected, the system immediately (1) presents the patient with crisis resources including the 988 Suicide & Crisis Lifeline (call or text 988), Crisis Text Line (text HOME to 741741), and 911 emergency services; (2) halts the AI conversation to prevent further engagement during a crisis moment; and (3) sends an alert to the supervising therapist for clinical follow-up.
Content Safety: The AI system is designed to never encourage, facilitate, or produce content related to self-harm, suicide, substance abuse, or other harmful behaviors. All AI interactions are supervised by licensed therapists who configure the AI’s therapeutic approach, review session summaries, and maintain clinical oversight.
Not a Replacement for Emergency Services: ExtendedSessions.ai is not monitored in real time and is not a substitute for emergency mental health services. All patients acknowledge this during onboarding and are instructed to call 911 or the 988 Crisis Lifeline if they are in crisis.
Clinical Oversight: Every patient is connected to a licensed therapist who supervises their care, configures the AI’s therapeutic approach, and reviews session summaries. The AI operates as a support tool directed by the therapist — it does not provide independent clinical judgment.
